ChiroSpring's intake forms are now tied closely to patient appointments, which means getting intake forms done just got a lot easier!

  • Auto-generate intake form requests for new appointments
  • Filled-out forms are always tied to the correct DOS
  • Send your patient a personalized list of forms to fill out

  • Monitor each incoming patient’s intake forms progress in the Front Desk

Step 1: Link intake form templates to an appointment category


Step 2: Create an appointment.  The form requests are auto-generated for that appointment category! This happens for online scheduling too.

Step 3: Go to Communicator dashboard and send the link to the patient (SMS or email).  You can also edit the list of forms requests.

Step 4 : Patient opens a personalized list of forms to fill out!

Step 5: Monitor your patients' progress in the Front Desk and follow up.
