Patient Documents

The Documents section allows you to securely store patient-related files directly within the patient's account. Instead of searching through paper records or external storage, you can keep important documentation in one central location that's easy for your staff to access whenever it's needed.

Whether you're uploading X-rays, lab results, intake forms, insurance documents, or patient identification, the Documents section helps keep records organized and readily available throughout the patient's care.

You can upload files into built-in document categories, including:

  • X-ray Records
  • Patient Information
  • Patient ID Cards
  • Patient Photographs - (used as the patient's profile photo throughout ChiroSpring, including dashboards, widgets and other areas)
  • Medical Records
  • MRI Results
  • Test Results
  • Lab Reports
  • Advance Directives
  • Do Not Resuscitate (DNR) Orders
  • Durable Power of Attorney
  • Living Wills
  • CCD/CCR Files
  • Patient Intake Forms

If the default categories don't meet your needs, you can create Custom Folders to organize documents in a way that makes sense for your practice. For example, you might create folders for:

  • Functional Medicine
  • Nutrition
  • Weight Loss
  • Legal Documents

Custom folders can be:

  • Created
  • Edited
  • Deleted

Important: Deleting a custom folder permanently removes the folder and every document stored within it.

Deleting Custom Folders

Custom folders can be removed from either the Patient's Documents page or from the Custom Folder page.

Delete from the Patient's Documents

When deleting a folder from a patient’s Documents page, you can choose to:

    • Remove it for This Patient – Removes the folder and its documents for the selected patient only.
    • Remove it for All Patients – Removes the folder practice-wide and deletes all documents stored within that folder for every patient.

Delete from the Custom Folder page – Removes the custom folder for the entire practice and permanently deletes all documents contained in that folder for all patients.

Best Practice: If a folder is no longer needed for a single patient, delete it from that patient's Documents page. Only delete a folder from Custom Folders if you're certain it is no longer needed anywhere in your practice. When possible, consider archiving the folder instead of deleting it.
