Fee Slip - How to Create a Claim

Creating a claim in ChiroSpring is a simple process. It involves starting with the Fee Slip (the portion of the claim typically filled out by the provider). The Fee Slip, when saved, creates the claim.

What's great is all diagnosis and charges from the previous Fee Slip will ditto over (copy from last) from the previous Fee Slip. This is a major time saver as the provider only needs to change what is different from that visit's diagnosis and procedures.


In this article, we will show you the process for creating a Fee Slip.


Before we get started, we want to point out an important setting. Some practices do not want the Fee Slip to be started until after the note is started (while other practices do want to allow this). Please make note of this setting (see image below).

How to Start a Fee Slip

ChiroSpring brings versatility to your practice allowing you the ability to create a Fee Slip through various paths in the software. Fee Slips may be started from Front Desk, EHR Menu, Treatment Dashboard and of course by starting and saving your treatment note.


Fee Slip - From Front Desk

From the front desk, click the circle next to "Note". Here you will be presented with your Treatment Note templates to start a note. In addition, you may choose to select "Create Fee Slip" to jump directly into the Fee Slip.


What's great is that you can always start the Fee Slip, if you prefer, and then go back and edit the fee slip to complete it. To do this, follow the same process. You will see new options in popup menu after a note and Fee Slip have been started.

Of note, if the Fee Slip is created before the note is started, when the note is started you will be prompted with an option to link the note to a fee slip. See below.

Now, once the note and fee slip are linked, when clicking the bubble on Front Desk (next to note) you will be presented with options to edit both the note and fee slip. See below.

The exact same process can be peformed by searching for your patient, then hovering over the EHR Menu. See below.

Fee Slip - From Treatment Dashboard

Many times the provider will want to review the patient before starting their Fee Slip or Treatment Note. For this, navigate to the patient's Treatment Dashboard. This can be done by clicking the hyperlink from Front Desk, or by searching for the patient and navigating to the Treatment Dashboard using the menu on the right.

The most common way to start a Fee Slip is by starting and saving your Treatment Note. As soon as the treatment note is saved, the Fee Slip will open (and ditto over from the previous visit's treatment note).

Completing a Fee Slip

Once you have started the Fee Slip through one of the processes above, you can go through the various sections of the fee slip to add your diagnosis, procedures (and even products which will not appear on a claim) as well as diagnosis pointing and modifiers.

  • Billing Details

This section will display the patient's Insurance Payer, Membership or Non-insurance. You can also track your Treatment Plan in this top Billing Details section.


Of note, you may only use a Fee Schedule on your patient for "Non-insurance". Fee Schedules should not be confused with Procedure Allowances (Allowed Amounts). Procedure Allowances are for insurance, and are the amounts the insurance allows on a given procedure. Fee Schedules are discounts for cash (non-insurance) procedures.

  • Diagnoses

Select diagnosis codes

    • Search for your code by regions, favorites, patient (all past diagnoses) or full list.
    • Set onset date
    • Reorder diagnosis
    • The Magic Wand ðŸª„is a Time-Saving Tool. When clicked it adds the top four diagnoses automatically to your procedure selections. It can be used on the procedure tab as well.

  • Procedures

Select procedure codes

    • Search codes by lookup, favorites or billing groups.
    • Easily reorder procedures to match the treatment sequence.
    • Update units: Adjust the number of units for each procedure as needed.
    • Add Modifiers and Point Diagnosis

  • Product Sale

Add a Product to the Patients fee slip.

You may add a product to your Fee Slip. Products will not be part of the claim, but will be presented to your staff during checkout. As an example, if the provider is with the patient and doing the Fee Slip, and the provider knows they want to sell a product such as Biofreeze, they can add it here. To add a product click the green + (Add Product) button (see image above).


To add additinal products locate the product from the left and repeat the process.

  • Summary

The Summary section provides an overview of the Fee Slip allowing you one screen to see and interact with your selections. On repeat visits when the Fee Slip dittos over, it will default to this view. In many cases, no changes are needed from last visit to having the Summary View available can save you time.

    • Pro Tip: Use the Magic Wand here to automatically point top 4 diagnoses to all procedures.
    • Fee Schedules: Easily select a fee schedule per procedure*Non-Insurance Claims and Procedures only
    • Overrides: Make changes to fees as needed. *Non-Insurance Claims and Procedures only
    • User Earnings: Assign earning provider per procedure.
    • Units: Review and adjust the total number of units, including the option to specify time-based units (ex. 15-minute increments) for applicable procedures.
    • NDC Numbers: Enter the National Drug Code (NDC) for medications.

You will also see a Legend for Charged, Not Charged and No Insurance Bill (cash charges). For any procedure click the box (example below is a green check which means charged). When clicked the system will toggle through the three options.

  • Charged - The charge will appear on a claim and be billed to insurance
  • Not Charged - The charge will not appear on an insurance claim but will instead appear on a "Cash Claim". In addition, it will be $0.00.
  • No Insurance Bill - The charge will not appear on an insurance claim but will instead appear on a "Cash Claim". The price for the charge will be charged to your patient.

After a Fee Slip is saved, the new claim will appear in the pending claim folder. Claims will automatically be scrubbed and warnings for errors will be indicated for the user to review and correct by editing the claim.
